Impact of a High Body Mass Index on Lower Extremity Injury in Marathon/Half-Marathon Participants

Background:

To evaluate whether a high body mass index (BMI) predisposes marathon/half-marathon participants to lower extremity injuries.

Methods:

Consenting adult participants at the 2008 National Marathon to Fight Breast Cancer were enrolled in this observational study. The primary outcome measure was prevalence of self-reported lower extremity injury, during both training and race participation, with respect to BMI.

Results:

There were 194 subjects with complete data: 139 females (72%) and 55 males. Forty-six percent of females and 51% of males ran the full marathon (P = .63). Median BMI was 23.7 kg/m2 for females and 26.2 kg/m2 for males (P = .001). Eleven (24%) females in BMI tertile 1 (T1) suffered a training injury, while 9 (18%) from T2 and 4 (9%) from T3 suffered injuries (P = .072; OR 0.89; 95% CI 0.78 to 1.01). Twenty-six (19%) females suffered an injury during the race. Females in T1 were more likely to suffer a race-related injury (P = .038; OR 0.87; 95% CI 0.77 to 0.99). Females were 13% less likely to suffer a race-related injury with each 1-unit increase in BMI. Rates of injury did not differ by BMI tertile in males.

Conclusions:

A high BMI did not impart an increased risk of lower extremity injury during training or race participation.

Lower Extremity Muscular Strength and Leukocyte Telomere Length: Implications of Muscular Strength in Attenuating Age-Related Chronic Disease

Objective:

Leukocyte telomere length (LTL) shortening is characteristic of aging and is associated with morbidity and mortality, independent of age. Research demonstrates that lower extremity muscular strength is associated with mobility, morbidity and mortality; however, no study, to our knowledge, had examined the association between lower extremity muscular strength and LTL, which was the purpose of this brief study.

Methods:

Data from the 1999–2002 NHANES was used (N = 2410; 50–85 years). Peak isokinetic knee extensor strength (IKES) was objectively measured with LTL assessed from a blood sample.

Results:

After adjustments, for every 50 N increase in IKES, participants had a 9% reduced odds (P = .04) of being in the 1st (vs. 4th) LTL quartile.

Discussion:

Lower extremity muscular strength is associated with LTL, suggesting a possible mechanism through which lower extremity muscular strength may be associated with morbidity and mortality.

Lower Extremity Muscle Strength and Risk of Self-Reported Hip or Knee Osteoarthritis

Purpose:

The purpose of this study was to investigate the gender-specific longitudinal association between quadriceps strength and self-reported, physician-diagnosed hip or knee osteoarthritis (OA).

Methods:

Subjects were 3081 community-dwelling adults who were free of OA, joint symptoms and injuries, completed a maximum treadmill exercise test, had isokinetic knee extension and flexion and isotonic leg press strength measurements taken at baseline and returned at least one written follow-up survey. Multivariate logistic regression was used to estimate odds ratios and 95% confidence intervals.

Results:

Women with moderate or high isokinetic quadriceps strength had a significantly reduced risk (55% to 64%) of hip or knee OA. A similar, nonsignificant trend was noted among men. Moderate isotonic leg press strength was protective for hip or knee osteoarthritis among men only.

Conclusions:

These results suggest that quadriceps weakness is an independent and modifiable risk factor for lower extremity OA, particularly among women.

Associations of Leisure-Time Physical Activity With Balance and Lower Extremity Strength: A Validation of the Neuromuscular Part of the Physical Activity Pie

Background:

The importance of neuromuscular-type exercise (NME) has been recognized in recent recommendations for public health. However, the knowledge on associations and dose response of different types of leisure-time physical activity (LTPA) with musculoskeletal fitness and health is incomplete. This study evaluated the validity of the NME recommendation for public health introduced by the Physical Activity Pie.

Methods:

Engagement in LTPA and health-related fitness were assessed in 2 consecutive studies with the same adult population age 30 to 69 years (n = 575). Cross-sectional associations between different LTPA types and motor and musculoskeletal fitness were examined by logistic-regression models.

Results:

Engagement in NME was associated with good static and dynamic balance and lower extremity strength. The highest odds ratios (OR) were found between brisk NME and static balance (most vs least ft OR = 2.39, moderate vs least ft OR = 1.94) and brisk NME and leg strength (more vs least ft OR = 2.10). Some associations were also found between brisk aerobic exercise and good balance.

Conclusions:

This cross-sectional study suggests that the recommendation for NME in the Physical Activity Pie is valid in terms of balance and leg strength, the 2 major fitness factors related to mobility functioning, especially among aging adults.

Muscle Strength and Postural Stability in Healthy, Older Women: Implications for Fall Prevention

Background:

Effect of muscle strength and balance on falls has not been well researched in healthy older women. The purpose of this study was to compare lower extremity strength and balance in older healthy women during each decade of life and to investigate which factors are different in women with a history of falling.

Methods:

We retrospectively studied 240 women age 50-89 y. Measures of muscle strength, postural stability, and incidence of falls over the past year were obtained from client charts at Texas Woman’s University’s Health Promotion & Research Center from 1996 to 2002.

Results:

Strength declined significantly with age in all muscle groups except knee extensors. Age, hip flexor and abductor strength, and postural stability were significantly different in women who had fallen.

Conclusions:

Strength decline was not consistent across muscle groups. Women who were older, had less hip flexor or abductor strength, or less balance were more likely to have fallen.

Physical Activity and Physical Function in Older Adults With Knee Osteoarthritis

Purpose:

To assess correlates of physical activity, and to examine the relationship between physical activity and physical functioning, in 160 older (66 ± 6 years old), overweight/obese (mean body mass index = 33.5 ± 3.8 kg/m2), sedentary (less than 30 mins of activity, 3 days a week) individuals with knee osteoarthritis.

Methods:

Physical activity was measured with accelerometers and by self-report. Physical function was assessed by 6-min walk distance, knee strength, and the Short Physical Performance Battery. Pain and perceived function were measured by questionnaires. Pearson correlations and general linear models were used to analyze the relationships.

Results:

The mean number of steps taken per day was 6209 and the average PAEE was 237 ± 124 kcal/day. Participants engaged in 131 ± 39 minutes of light physical activity (LPA) and 10.6 ± 8.9 minutes of moderate-vigorous physical activity (MPA/VPA). Total steps/day, PAEE, and minutes of MPA/VPA were all negatively correlated with age. The 6-min walk distance and lower extremity function were better in those who had higher total steps/day, higher PAEE, higher minutes of MPA/VPA, and a higher PASE score.

Conclusions:

This study demonstrates that a population who has higher levels of spontaneous activity have better overall physical function than those who engage in less activity.

Validation of Questionnaire-Assessed Physical Activity in Comparison With Objective Measures Using Accelerometers and Physical Performance Measures Among Community-Dwelling Adults Aged ≥85 Years in Tokyo, Japan

Background:

To date, there is no physical activity (PA) questionnaire with convergent and construct validity for the oldest-old. The aim of the current study was to investigate the validity of questionnaire-assessed PA in comparison with objective measures determined by uniaxial and triaxial accelerometers and physical performance measures in the oldest-old.

Methods:

Participants were 155 elderly (mean age 90 years) who were examined at the university and agreed to wear an accelerometer for 7 days in the 3-year-follow-up survey of the Tokyo Oldest-Old Survey of Total Health. Fifty-nine participants wore a uniaxial and triaxial accelerometer simultaneously. Self-rated walking, exercise, and household PA were measured using a modified Zutphen PA Questionnaire (PAQ). Several physical performance tests were done, and the associations among PAQ, accelerometer-assessed PA, and physical performances were compared by Spearman’s correlation coefficients.

Results:

Significant, low to moderate correlations between PA measures were seen on questionnaire and accelerometer assessments (ρ = 0.19 to 0.34). Questionnaireassessed PA measure were correlated with a range of lower extremity performance (ρ = 0.21 to 0.29).

Conclusions:

This PAQ demonstrated convergent and construct validity. Our findings suggest that the PAQ can reasonably be used in this oldest-old population to rank their PA level.
