Patterns of Noncompliance in Adolescent Field-Based Accelerometer Research

Background:

The primary purpose of this study was to investigate patterns of noncompliance in an adolescent field based accelerometer study. A further purpose was to investigate the effect of a cost efficient strategy (SMS reminder message) on the compliance of adolescents

Method:

The research carried out in 2010 involved 117 second level students (12.41 ± .53 yrs) from 4 schools in a rural Irish town. The Actigraph accelerometer data were processed over 7 days to determine compliance level.

Results:

Students were more likely to remove their monitor in the evening period than at any other time, however if students removed their monitor after school it remained unworn for a significantly longer duration than in any other time period. Students who received a SMS message were significantly more likely (P = .008) to wear their monitor in the morning than those that did not.

Conclusions:

Sending an SMS message each morning is effective for improving the number of students wearing monitors to school. The after school period is a critical period for nonwear time and should be targeted in future studies wishing to improve compliance.

A Comparison of Engagement Strategies for Encouraging Outdoor Walking

Background:

This study investigated the effectiveness of enhanced cognitive awareness as a means of encouraging outdoor walking. An intervention using engagement-based strategies was compared with a more traditional walking intervention focused on developing and committing to a personalized walking schedule.

Methods:

117 adults were randomly assigned to 1 of 2 treatments—Standard Care (schedule setting, commitment) or Engagement (awareness plans)—and asked to take at least 3, 30 minute outdoor walks each week for 2 weeks. During the study period, self-report and objective measures were used to collect data on walking behavior.

Results:

Individuals in both treatment conditions reported significant increases (P < .05) in walking behavior. Participants in both treatments failed to sustain these increases at a follow-up measure 4 weeks later. However, the Engagement condition was particularly effective for those individuals who had less prior experience maintaining a walking routine.

Conclusion:

Overall, the findings suggest it may be beneficial to incorporate engagement-based strategies into existing walking interventions. Results of this study also raise the possibility that efforts to encourage cognitive awareness may make the outdoor walking experience more interesting and enjoyable.

Effectiveness of Recruitment Strategies for a Physical Activity Intervention in Older Adults With Chronic Diseases

Background:

Engaging sedentary individuals in physical activity (PA) is challenging and problematic for research requiring large, representative samples. For research projects to be carried out in reasonable timeframes, optimum recruitment methods are needed. Effective recruitment strategies involving PA interventions for older adults have not been determined.

Purpose:

To compare the effectiveness of recruitment strategies for a PA intervention.

Methods:

Two recruitment strategies, print media and personal contact, targeted health-care professionals and the general public.

Results:

The strategies generated 581 inquiries; 163 were randomized into the study. Advertising to the general public via print materials and group presentations accounted for 78% of the total inquiries. Referrals from physicians and health-care professionals resulted in 22% of the inquiries.

Conclusion:

Mass distribution of print material to the general public, enhanced by in-person contact, was the most effective recruitment strategy. These findings suggest various recruitment strategies targeting the general population should be employed.

Measuring Physical Activity Self-Regulation Strategies in Older Adults

Background:

Theoretically, self-regulatory strategies (eg, goal setting, self-monitoring) are an important influence of behavior change, but very little research has examined the relationship between self-regulation and physical activity (PA) behavior. Petosa’s (1993) 43-item PA self-regulation scale (PASR-43) affords the opportunity for studying this construct in the context of PA; however the PASR-43 has not been tested for structural aspects of validity. Therefore, this study examines the structural validity of the PASR-43 in older adults.

Methods:

The structural validity of the PASR-43 was tested in a large sample of older adults from North and South Carolina and Ohio (N = 460) using maximum likelihood estimation and confirmatory factor analysis in AMOS 5.0.

Results:

The original 6-factor model for the PASR-43 scale did not represent an acceptable fit to the data (x2 = 4732.25, df = 845, P < .0001, RMSEA = 0.10, NNFI = 0.67, CFI = 0.71). Based on a post hoc specification search, iterative model modifications resulted in a 12-item PA self-regulation scale (PASR-12) that represented an excellent fit to the data (x2 = 70.75, df = 39, P = .001, RMSEA = 0.04, NNFI = 0.98, CFI = 0.99).

Conclusions:

The PASR-12 provides a concise and valid measure of PA self-regulation for use with older adults. Future studies should cross-validate the PASR-12 and examine invariance across time and between age, ethnic, gender, and geographical groups.

Some Strategies for Sustaining a Walking Routine: Insights From Experienced Walkers

Background:

This study explores the strategies that experienced walkers felt were most useful for sustaining outdoor walking routines. To investigate this issue, a survey-based instrument was used in combination with a Conceptual Content Cognitive Mapping (3CM) exercise.

Methods:

Seventy-one experienced walkers were asked to complete the 3CM exercise to explore the strategies that have helped them regularly walk outdoors. After 1 week these same individuals received a survey investigating these same issues as well as demographics and physical activity participation.

Results:

There was general agreement between 3CM and survey data with respect to the strategies used by experienced walkers. The most highly endorsed strategies involved using health goals and supportive walking environments. Survey results also revealed that those more likely to endorse the use of social support took fewer walks per week, but engaged in more nonwalking related physical activity.

Conclusions:

Overall, the findings suggest that experienced walkers use a variety of strategies. Strategies such as focusing of the positive health outcomes, using attractive natural settings, and developing realistic action plans appeared to be the most useful. These results also indicate the 3CM technique may be an effective way to explore beliefs and motivations regarding physical activity.

Strategies for Developing and Delivering a Parental Physical Activity Intervention: Answers to the What and How

Background:

Parents are at risk for physical inactivity; however, few studies have designed physical activity (PA) interventions specifically applied to individuals with young children. To ensure the effectiveness of interventions, it may be useful to first elicit the needs from the target population and incorporate salient strategies identified to the design and delivery of a resultant intervention. We aimed to explore strategies for what to include in and how to best deliver a program designed to increase parental PA.

Methods:

Twelve parents (6 mothers, 6 fathers) of children younger than 5 years participated in focus group discussions exploring strategies for an intervention program designed to increase parental PA.

Results:

A range of themes such as Focus on the Children and Flexible Life/Family Plans imbedded in strategies such as persuasion and information, problem-solving, skill building, and environmental approaches were identified. In addition, a range of strategies for how to best deliver a parental PA intervention evidenced in emerging themes such as Diverse and Brief and Individualized Approach was discussed.

Conclusions:

Future research should continue to adopt a ground up, community-based approach to the development and implementation of interventions for this at-risk group to ensure sustained involvement in regular PA.

Impact of Lifestyle Strategies on Longer-Term Physical Activity and Weight

Background:

Lifestyle interventions promote increased physical activity (PA) and weight loss; however, relapse to sedentary behavior and weight regain are common.

Methods:

We analyzed baseline and 24-month data from participants in the Slow the Adverse Vascular Effects (SAVE) study. SAVE included an 18-month behavioral intervention. At 24 months, participants completed a survey about lifestyle strategies used in past 6 months. PA levels were assessed with the Modifiable Activity Questionnaire. We compared change in weight, BMI, and PA from baseline to 24 months by use of strategies vs. no use.

Results:

214 participants (61%) completed 24-month visit. 74% were female and 86% were white. At 24 months, 65% used self-monitoring, 67% group/commercial support, 94% other behavioral skills, and 27% used professional support within past 6 months. At 24 months, participants who used self-monitoring (5.2 vs. –0.8 MET-hr/wk; P = .001) and group/commercial support (4.3 vs. 0 MET-hrs/wk; P = .01) had greater PA increases compared with those who did not use strategies. Participants who used other behavioral strategies had a significantly greater percent decrease in weight than those who did not.

Conclusions:

Of the lifestyle strategies used following intervention, self-monitoring and group/commercial support may be particularly important in longer-term PA levels.

Should Physical Activity Intervention Efforts Take a Whole Population, High-Risk, or Middle Road Strategy?

Purpose:

This study was designed to compare theoretical strategies for changing physical activity (PA) in terms of their potential to reduce the incidence of chronic conditions in midage women: (1) whole population: +30 minutes/week in all, (2) high-risk: +60 minutes/week in the lowest 25% of the PA distribution, and (3) middle road: shift all those not meeting guidelines to a level commensurate with meeting guidelines.

Methods:

10,854 participants (50–55 years in 2001) in the Australian Longitudinal Study of Women’s Health completed mail surveys in 2001, 2004, 2007, and 2010. PA was calculated as MET·minutes/week spent in walking, moderate and vigorous PA in the previous week. Incidence rates per 1000 person-years for diabetes, heart disease, hypertension, cancer, and depression were calculated for the actual distribution and after modeled shifts in PA.

Results:

The incidence rates were 10.6 for diabetes, 7.0 for heart disease, 30.7 for hypertension, 8.0 for cancer, and 28.4 for depression. Greater reductions in incidence were found for the middle road strategy than for the whole population and high-risk strategies, with reductions ranging from –6.3% for cancer to –12.3% for diabetes.

Discussion:

This theoretical modeling showed that a middle road strategy to increasing PA was superior to the whole population and high-risk strategies, in terms of reducing incidence rates of chronic conditions in middle-aged women.
